Introduction

Course overview

Organisations that investigate incidents well learn from them and prevent recurrence. Poor investigations blame individuals, miss systemic causes and allow the same events to happen again.

This Aractech course teaches a structured approach to HSE incident investigation: initial response, evidence collection, witness interviews, timeline reconstruction, root cause analysis using recognised techniques, human factors, corrective actions and report writing. Participants investigate a realistic case from start to finish.

The course builds skills that support both learning and legal defensibility.
